MADISON, Wis. – The Wisconsin men's basketball team is looking to add a walk-on to its roster for the immediate 2022-23 season and will host an open tryout for eligible enrolled University of Wisconsin-Madison students on Wednesday, Oct. 19 at 5 p.m. in the Nicholas-Johnson Pavilion.

Interested individuals must be current full-time students at UW-Madison and must fill out and complete the following/attached forms by 4 p.m. on Friday, Oct. 14.

All participants must fill out the attached and following Open Tryout form, Assumption of Risk form, Health Insurance Information form, and Preparticipation Physical Evaluation (PPE) form. All participants must fill out Page 1 of the PPE form. If participants have had a physical examination within six months of the tryout date, they must provide documentation. If participants have not had a physical examination within six months of the tryout date, a physician will need to fill out and sign Pages 2-4 of the PPE form.

• Health Insurance Information form
• Preparticipation Physical Evaluation (PPE) form
• Copy of insurance card (both front and back of card)

You must fill out, complete and submit requested forms in order to attend the tryout!

This is the first time the Badgers have held an open tryout since 2019 when Samad Qawi joined the roster. UW also added Jackson Bax in 2015, J.D. Wise in 2009, and Wqinton Smith in 2007 via open tryout.
